THIS NOTICE DESCRIBES HOW YOUR MEDICAL INFORMATION MAY BE USED AND DISCLOSED, HOW YOU CAN ACCESS THAT INFORMATION, AND YOUR PRIVACY RIGHTS. PLEASE READ IT CAREFULLY.

Who This Notice Applies To

This Notice of Privacy Practices (“Notice”) explains how Solara Autism Services (“Solara,” “we,” “our,” or “the Company”) protects and uses your protected health information (PHI). It applies to all Company employees, clinicians, contractors, volunteers, and other individuals involved in providing your care who are authorized to access your medical information.

These individuals are required to follow the terms of this Notice and may use or disclose your protected health information for treatment, payment, healthcare operations, or other purposes permitted or required by law.

Protected health information includes information that identifies you and relates to your past, present, or future physical or mental health, healthcare services, or payment for healthcare services.

Our Commitment to Protecting Your Privacy

We understand that your medical information is personal and are committed to protecting your privacy.

To provide high-quality care and comply with federal and state laws, Solara creates and maintains records of the services you receive. This Notice describes how we may use and disclose your medical information, your privacy rights, and our legal responsibilities regarding that information.

By law, Solara is required to:

  • Maintain the privacy and security of your protected health information.
  • Provide you with this Notice describing our legal duties and privacy practices.
  • Follow the terms of the Notice currently in effect.
  • Notify you if a breach occurs that compromises the privacy or security of your unsecured protected health information.

Both federal and state laws protect your health information. When federal and state privacy laws differ, we will comply with the law that provides you with the greatest level of privacy protection.

  • Coordinate care with other healthcare providers.
  • Share information with individuals involved in your continuing care after you leave our services, when permitted by law.

Payment

We may use and disclose your medical information to obtain payment for services provided.

For example, we may:

  • Submit information to your health insurance plan, Medicare, Medi-Cal, or other payers.
  • Verify insurance coverage.
  • Obtain prior authorization for recommended treatment.

Healthcare Operations

We may use or disclose your medical information for activities necessary to operate our organization.

Examples include:

  • Quality improvement
  • Staff training and supervision
  • Licensing and accreditation
  • Compliance activities
  • Audits
  • Contract administration
  • Legal matters
  • Performance evaluations

Facility Directory

If applicable, we may include your name, location, general condition (excluding specific medical details), and religious affiliation in our patient directory unless you object.

Family Members and Others Involved in Your Care

Unless you object, we may share relevant medical information with family members, close friends, personal representatives, or others involved in your care or payment for your care.

We may also notify these individuals of your location, general condition, or death when appropriate.

Appointment Reminders and Health Information

We may contact you with:

  • Appointment reminders
  • Information about treatment options
  • Health-related services or benefits that may be of interest to you

Communications will be made using methods permitted by law.

Emergencies and Disaster Relief

During emergencies or disasters, we may disclose your medical information to public or private organizations assisting with disaster relief so your family or others responsible for your care can be informed of your condition or location.

If you are unable to express your preferences, we will use professional judgment to determine whether the disclosure is in your best interest.

Fundraising

We may use limited contact information, such as your name, address, and telephone number, to inform you about Solara-sponsored events or fundraising activities.

As Required by Law

We will disclose your medical information whenever federal, state, or local law requires us to do so.

Workers’ Compensation

We may disclose medical information as necessary to comply with workers’ compensation laws and similar programs.

Public Health Activities

We may disclose medical information for public health purposes, including:

  • Preventing or controlling disease, injury, or disability
  • Reporting deaths
  • Reporting child, elder, or dependent adult abuse or neglect
  • Reporting adverse reactions to medications
  • Reporting defective products or product recalls
  • Notifying individuals who may have been exposed

DATA SHARING

Solara Autism Services is committed to protecting your personal information and respecting your privacy.

  • No Marketing Data Sharing. We do not sell, rent, or share customer information with third parties for their promotional or marketing purposes.
  • Mobile Information Protection. Mobile phone numbers, SMS opt-in information, and consent records are never shared with third parties for any purpose. Any references elsewhere in this Notice to permitted information sharing do not include mobile opt-in data or SMS consent.

SMS MESSAGING TERMS AND CONDITIONS

Solara Autism Services Respects your privacy. By opting into our SMS messaging service, you agree to the following terms regarding how we handle your data

Data Collection: We will collect your name, email address, mailing address, and mobile phone number when you sign up for SMS updates.  The information will be collected via the website contact form, email, rental agreement, or third-party reservation systems.

Data Usage: We use your data solely for sending you General conversational messaging to answer questions, schedule reminders and provide support to customers.


Data Security: We protect your data with secure storage measures to prevent unauthorized access.

Data Retention: We retain your information as long as you are subscribed to our SMS service. You may request deletion at any time.

MESSAGE AND DATA RATES MAY APPLY. Your mobile carrier may charge fees for sending or receiving text messages, especially if you do not have an unlimited texting or data plan.

Messages are recurring, and message frequency varies.

Opt-Out: You can opt out of the SMS list at anytime by texting, emailing, or replying STOP or UNSUBSCRIBE to info@Solaraaba.com or (833) 577-7222.  After unsubscribing, you will receive a final SMS to confirm you have unsubscribed and we will remove your number from our list within 24 hours.

Non-Sharing Clause: Mobile information will not be shared with third parties/affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties or affiliates. Information sharing to subcontractors in support services, such as customer service providers, is permitted.
